Preface

The First Affiliated Hospital of Zhengzhou University Organ Transplant Center is the leading hospital of many organ transplant hospitals in Henan, with liver and kidney transplant volume in the forefront of the country for many years. Often the liver donor comes in the middle of the night, and the source of the organ is unknown. In partner with Shanghai Renji Hospital, which was a major unit in live organ harvesting in 2018, the hospital established the national regional medical center for organ transplantation in Henan, seeking the great development of organ transplantation.

I. Hospital Overview

The First Affiliated Hospital of Zhengzhou University became a provincial and ministerial hospital in 2012. It is a third level first class, large comprehensive hospital, which is also the largest hospital in China. There are 95 clinical departments and 239 hospital wards with nearly 10,000 beds. The hospital has 12,409 working employees, 11,168 medical and health care technicians, including 523 with senior titles, 899 with associate titles, 4,733 with intermediate titles, among which 1,647 with doctoral degrees and 2,727 with master's degrees; 1 academician and 22 specially appointed academicians[1].
 

II. Organ Transplant Center

The Organ Transplant Center of the hospital is an advantageous key department. The director of the Transplant Center, Zhang Shuijun, was promoted to be the deputy secretary of the Party Committee of the hospital. The number of liver and kidney transplants in the transplant center ranks among the top hospitals in China, among which the number of pediatric kidney transplant cases ranks the Number 1 among domestic hospitals for four consecutive years. The First Affiliated Hospital of Zhengzhou University is also the organ transplant center of Henan Province[2]. In 2017, the First Affiliated Hospital of Zhengzhou University ranked third in the country in terms of liver transplantation, fourth in kidney transplantation, and children's kidney transplantation ranked first in the country for 4 consecutive years[3].

1. Kidney Transplantation Department 

The department includes specialized outpatient clinic, inpatient department with tissue matching laboratory and hemodialysis center. The number of kidney transplants has been the first in Henan Province for many years. In 2015 it completed 230 cases of kidney transplants, which ranked the fifth in China.

The department has a team of more than 40 staff members of transplant physicians, nurses, examiners and organ donation coordinators, including 7 physicians, 2 professors, 2 associate professors and 2 postgraduate instructors. There are 47 open beds and 10 monitoring beds in the department[4].

The children’s kidney transplantation in the First Affiliated Hospital of Zhengzhou University ranks first in the country for many years in a row. In 2021, the Kidney Transplant Center completed 109 cases of children's kidney transplants, making it the first children's kidney transplant center that completed more than 100 kidney transplants in a single year[5].

2.Hepatobiliary and Pancreatic and Liver Transplantation Surgery

The Department of Liver Transplantation Surgery is the liver transplantation center in Henan Province. The hepatobiliary-pancreatic and liver transplantation surgery actually operates more than 450 beds. There are 18 professors, 10 associate professors and more than 30 attending physicians, all with doctoral or master's degrees[6].

3. Media reported that the transplant volume in a single day is amazing

Dahe Healthy Newspaper reported on March 27, 2014: the hospital's Liver Transplant Center completed 4 liver transplants successfully on the same day, for 2 patients with liver cirrhosis and 2 with liver cancer.[7]

According to a report on Henan Daily in 2019: From March 26 to 27, a total of 13 organ transplants were completed in the River Hospital area of Zheng University First Affiliated Hospital, including 8 kidney transplants, 3 liver transplants and 2 lung transplants[8].

4. Super-short waiting time for transplantation, emergency transplantation

According to the report from the hospital’s official website, that on March 4, 2020, Professor Zhao Gaofeng of Thoracic Surgery as leading surgeon, working with the team of Anesthesiologists led by Professor Wang Yong, the team of ECMO led by Professor Li Jun, and the organ transplantation team from the operating room, completed the hospital’s fourth case of emergency rescue operation of lung transplant surgery. The patient was admitted to the hospital on February 29, 2020, and a matching donor lung was found in Qingdao, Shandong, and the donor organ was harvested at 5 am on March 4, 2020[9]. That is to say, before this operation, the hospital has completed 3 cases of emergency lung transplantation.

According to the report from the official website of the Zhengzhou University, that in April 2020, the First Affiliated Hospital of Zhengzhou University completed three cases of emergency lung transplantation within 24 hours[10].

III. Establishment of National Regional Medical Center for Organ Transplantation in Henan

On March 22, 2018, the Branch of Central China Organ Transplantation Center was inaugurated at the East Campus of the First Affiliated Hospital of Zhengzhou University. The center is jointly built by two hospitals, the First Affiliated Hospital of Zhengzhou University and Shanghai Renji Hospital.

Henan is a province with large population (having abundant organ donors and recipients), but relatively insufficient high quality medical resources. Shanghai Renji Hospital, on the other hand, is a leader in organ transplantation in China (but lacks organ donors and recipients). As the leading hospital for organ transplantation in Henan Province, Zhengzhou University First Affiliated Hospital is also at the forefront of liver and kidney transplantation and surgery volume in China for many years. The combination of the two hospitals, with complementary advantages, is strongly promoting the development of a national regional medical center for organ transplantation in Henan Province[11].

IV. Telephone Investigation:

Summary :Over a thousand cases of liver transplant; donor liver often arrived in the middle of the night so liver transplant conducted overnight; can be done if paying more money; live organ harvest; donor aged about 40 years old; transplantation continued during the pandemic.

“The patients come here at night and we do the liver transplantation surgeries overnight. That is very common here.”

Summary:

1. This year what we have done will break two hundred cases very soon, the case done last Saturday is the one hundred and ninetieth case.

2. Someone who came from Shenzhen, Guangdong, said our hospital has more liver source here, probably this is true. The more the liver donors available here, the shorter waiting time will be.

3. As long as our hospital has applied for and the liver becomes available, we do it even if it’s in the middle of the night. Most of the surgeries are done in the middle of the night, because the liver comes in the mid-night. If you only do it during daytime working hours, how can you do so many surgeries? Our professors here just do it whenever a liver becomes available!

4. Last year, a new hospital was opened in the new district of the East Zhengzhou, and it is estimated that the liver transplant can be carried out in the East district next month.

Professors do surgeries on both hospitals. They all run between two sides.

5. The corridor and stairwell in the old district are full of extended beds, too many patients, not enough beds to stay! Anyway, now a ward with regular and extended beds can basically receive more than a hundred patients.

Summary: Last year there were more than 200 cases, this year there have been 110 cases already. Type A (blood donor) is not easy to find. The estimated waiting time is about two weeks to one month.

Many of the directors in Shanghai have cooperation with us, because their patients can't wait too long to get the liver donors. They all bring their patients to Henan to do it. The nationwide rank of transplant volume: the first one is Renji, the second one is Zhejiang University, the third is us, the fourth may be Xiangya. Tianjin First Central Hospital is the largest one, however it has no data, because they have a part of data that is not released, so there is no way to know how many (cases) it conducted after all.
